  • What is your policy on payment?
    Payment is expected at the time of your appointment. Cash, Apple Pay, VISA, MasterCard, Discover and American Express credit cards are accepted. If your insurance carrier is accepted, it will be billed. If you are using out-of-network benefits with your insurance carrier or are self-pay, payment in full is required at the time of service, unless other arrangements have been made in advance.
  • What is your cancellation policy?
    We require at least 2 business days notice for cancellations. If you fail to cancel 2 business days prior to your scheduled appointment time or do not show up for your session, you will be charged a $200 fee.
  • What is your policy on confidentiality?
    Your patient records and content of sessions are strictly confidential. For this reason, no information concerning you as a patient is released without your written consent. Disclosure of information to anyone, such as an attorney and/or a family member must be requested by written authorization by the patient. In an emergency situation where you, the patient, are at imminent risk of death or serious medical consequence, I will release the critically relevant information to assist in preventing dire medical consequences that may result if that relevant information is not released.
  • How do you handle prescription refills?
    If you need a prescription refill, please contact me directly and I will send in the order to your pharmacy of choice. Prescriptions will be sent in only for current patients. Medication management practices and issues must be addressed during regularly scheduled appointments. Medication management appointments are dependent on the prescription and patient in question. If a client neglects or continues to postpone follow-up appointments, refills will be provided only after the patient is seen in a follow up session. Please keep in mind that prescription refill requests and non-urgent matters must be addressed during business hours.
